Humans and Bread

Food is a basic need for the creatures in the world to sustain their daily body methodology. Bread is a regarded as a symbol for life, symbol for prosperity and livelihood from ages. Bread, a food item was discovered primarily in the Middle East and the features of the bread were modified according to the regions it has been introduced. The bread was related to the divine source provided by the eternity. Since wheat is the cultivated in most part of the world and the bread is prepared with wheat. Bread therefore symbolizes food in various forms.Consumption of bread was known in the pre Christian era. In the initial times when bread was introduced to the English speaking countries the villages used to have to community kitchens where the women in every house used to bake their cakes at a common place. The women were regarded as the significant part in the family who nurture the family and their congregation was regarded as significant social involvement. The guests were welcomed in to the houses with a piece of bread and liquor to wish them a better life.The Russians regards bread and vodkha as a sacred combination. The bread which was prepared from wheat was considered basic food. Bread was used as a commodity in Egyptian ages. After the post harvest period people were provided with work and paid bread. Thus bread is considered as money. Bread or wheat was considered as basic ingredient for the livelihood and the rulers were at times conscious about the supply of the grains or bread to huge population in the kingdom.The bakers who tried to adulterate the bread making process were severely punished. The English used to introduce wheat cultivation and bread making in different parts of the world. Bread making even now is a daily routine affair for many families in different countries as it symbolizes a tradition, a habitual action. Now a days people in different parts prepare bread to suit their tastes to make it more nutritious and to include their and love and affection to share with family.

My host family were consisting of the old lady that’s name Peggy and she were hosting two students before we arrived there. She’s very kind women, Towards the middle of my spring vacation, We were looking for american high school for next semester. But poor us, most of the schools got fully enrolled for next semester. So we tried to contact the rest of the schools that we knew from our friends, relatives or family. And finally we found out that one of the school that’s called â€Å"The newman high school† still opening for enroll next semester. Lucky us that Peggy used to be the principle of the language school before, so she suggested us a lots of stuffs to prepare. We have been going to the News burry street, Havard university as long as I remembered. We know the best places for sight seeing. Where all the best rides , which park is the best. On day though while we were planning our next trip we decided that which days will we going back to EF. And we decided to stay in Boston for four more days in the hotel. It was a wonderful experience that we went there, walking around the city with my friends having just a map in my rucksack. Even though my summer was loaded with activity, it still seemed like a long break. After spending so much time in Boston, chilling around, and attending a guitar competetion , I felt like I needed more vacation! It was a wonderful experience and I learned a lot . Analysis of Generalization on the Example of Radio Shack’s Audience Radio Shacks Sociocultural Environment Radio Shack is a computer-controlled radio that has a recording feature on live programs. It is available in both the United States of America and Canada. The aspect of socio-culture for both countries is different in the sense that, the USA is diverse with many ethnic groups while Canada lacks diversity since it has few ethnic- groups. The culture of a place is typically influenced by the number of ethnic groups that exist. For instance, the United States and Canada are different when it comes to market structure and consumption. For both countries, there is a notable variation, which can be attributed to different cultures. In the USA it needs to incorporate the diversity to a large existent while in Canada it is not necessary. As such, the Sociocultural environment is seen as fundamental in coming up with generalized decisions as well as specific operating needs of each sociocultural cluster (Macky et al., 2008). Geert Hofstede looks at the sociocultural environment as a different way, which individuals of one group are distinguishable from the other. This is a result of how they have adapted to their society and how they can view and react to various elements (Hofstede, 2011). Socio-cultural parts of the States include considering a product on merit, that is, how it meets the need it was purchased for and the cultural aspects of the company involved. Moreover, the company can consider taking part in various social responsibility programs. Corporate social responsibility programs are essential in that; they can help market the products of the company in their localities. Radio Shack is one company that is surviving in different cultures, however, Canada, on the other hand, is more of how collectively a product can meet the desired results. In this case, it is not about a single person but the entire group. Similarly, the corporate social responsibility that the parent company is involved in p lays a role in improving Radio Shack penetration in the market. The socio-culture of the USA is first centered on individualism, which ideally means every person wants a product or a service that perfectly meets their needs and many of the consumers do not consider the tastes of others (Apsalone Sumilo, 2015). As such, Radio Shack is meant to have personal satisfaction. Concerning masculinity, the States is not that divided when it comes to roles, ideas or products that befit a certain gender. From a business perspective, Radio Shark can easily qualify as a unisex product that cuts across the needs of both men and women. Uncertainty in the USA is high because it is tolerant to various ideas and allows businesses to try products without any restrictions. To Radio Shack this is a good environment for trying out new products, as the market allows a variety of products without having to judge and condemn them in totality. Canada, on the other hand, is more into collectivism, mainly because, it has less ethnic groups compared to the USA. As such, there are strong groups such as extended family connections or influence by church groups at different age levels (Dube et al., 2015). Radio Shack has to be spot on when it comes to its programming of a product in that this market takes only a single individual to affect the sales to a huge group due to the collectivism, which represents socio-cultural aspects. The other aspect of masculinity in Canada is pronounced in the sense that men and women tend to be competitive and assertive. As such, for the business, a product has to be tailored to adapt to the competitive nature of both genders. Uncertainties in Canada are very low, in that, the level of tolerance that is accorded to ambiguity is so minimal. Rules and regulations are adhered to strictly as well as the need for safety and security of consumers. Radio Shack’s business in this market is therefo re subject to strict controls from the regulatory authority, and there is no room for any errors. In the business world, there are situations where uncertainties are unavoidable either due to political or economic causes. However, the social-cultural aspect of inequality in both countries is similar. Every person or business is accorded same opportunity to access government services or social amenities. This has been key for Radio Shack in the sense that, venturing into a foreign market comes with a lot of rules and regulations with the outstanding aspect of foreign ventures. Operating business in a foreign land can be difficult, especially when it comes to compliance. Radio Shack has enjoyed relatively a pleasant business season in both cultures. This can be attributed to their mode of operation, which incorporates diversity. In both Canada and USA, the market is friendly for the company in many ways. As such the aspect of equality means ease of accessing market as well as launching the product. The socio-cultural factors have a huge impact on the business of Radio Shack in various ways. The aspect of individualism means that the companys marketing structure need to focus independently on every customer. The reverse is true for the collectivism aspect in that, all required is a good marketing structure at the group level and the product will be accepted. Design of a product is also affected by the element of masculinity in that; in an environment where these aspects are more pronounced, the product should equally make them pronounced. This can be achieved by having labels that distinguish between male and female. The level of tolerance to uncertainty also plays a role. Unfamiliar products that are not from the individual’s country may be unwelcome, and this may affect business negatively. As such, cultures that have low uncertainty tolerance pose a high risk to a business regarding having a competitive edge in the market or not.

Livingstone was initially a missionary. He began his explorations not so much as an attempt to map uncharted territory, but more as an effort to open up new paths to commerce and Christianity. While working to construct missionary stations deeper into the "Dark Continent's" interior, he came into close contact with Africa's slave trade. His intention was then to slowly eradicate the slave trade by replacing it with the trade of European goods. He believed that "civilization" must be brought to Africa and that Christianity and commerce were the perfect carriers. Livingstone was not very successful as a missionary and made numerous geographical errors (Sykes 1996). After all, he converted only one African who later reconverted and various miscalculations nearly sacrificed his Zambezi expedition. Later he thought he had found the source of the Nile only to realize that it was actually the upper Congo (Sykes 1996). Still, he represents for many the spirit of the explorer. His contributio ns to the geography of Africa are invaluable. It was he, after all, who first began to draw attention to Africa and her people. Carl Sauer is considered one of the founding fathers of American geography. He marks the initial separation of physical geography from human or cultural geography. After overhearing many teenagers talking about why they struggle with paying attention early in the morning, it provided a good understanding for the problem. 4. Twenty well-controlled experiments published in prestigious journals strongly suggest a cause for why teenagers have more difficulty focusing their attention early in the morning than do younger children. 5. After a recent airing of a story on the news show Dateline on NBC about some people being morning people and others being night people, Channel 5 asked viewers to call in the reasons they have difficulty focusing attention in the morning. The decision to give the aforementioned reasons the rank in which they received was based fully on the evidence presented in each statement. Items 1-3 indicated a true correlation between the proposed question and the stated reason and personal understanding. Item 6 was not a source focused on the study of teenagers, but rather its research was done on a PSYCHOLOGY 3 broad level. Finally, item 7 did not come from someone with credibility nor did it possess any scientific verification to back up any reasoning. Also, it did not contain a valid reason pertaining to the question at hand. There were only two reasons suggested that contained factual evidence that were believable and credible (Shohov, 2004). Items 1 and 2 proposed that research in relation to the proposed question had been carefully and scientifically performed. Item 1 declared that thirty well-designed studies had been performed concluding that a specific hormone was believed to be the cause for a teenager’s inability to focus early in the morning. These studies had also been published in prestigious journals. Item 2 aired on a publically respected early morning news broadcast. The finder-of-fact was an expert nutritionist who had conducted many interviews on children of all ages. Her conclusions were drawn based on answers voluntarily given by the interviewed teenagers. Her findings concluded that teenagers skipping breakfast had more difficulty focusing early in the morning. The new owners wanted the newly-acquired company to grow even further. The first few months seems to have been a success Marcus spent a lot of his time with employees and customers, learning the field since he was, as mentioned in the text, an outsider. However, keeping the companys integrity and image intact was a priority. However, the problems outlined in the case seems to start or at least have been worsened with what will become CMRs largest account, Blackstone Homes. Problem Identification After reviewing the text in question, three major problems stand out: 1) Customers change requests: When Blackstone Homes sell a house, it does of course! come with a pre-finished and installed kitchen, included in the price. However, if the customer wishes, he or she would visit CMRs showroom and they would build these woodwork according to their new specifications. The added cost will then be charged to the contractor, which will later add it to their bills. This takes time and lead to price increases. This is, in part, what contributed to the clash with Blackstone Homes. 2) IT solution not adapted: InfoCentral, the software implemented by Marcus, does not work out as intended and do not fit the business needs. It is not always updated as required by the employees. According to the text, it may be helpful for the commercial and financial/accounting side, but is lacking several features for its residential counterpart to be really as useful as it can be. 3) Deteriorating business relationship The business relations between the two companies started well and was fulfilling for both part. When the business expended, so did the problems. The root cause seems to be the change requests made by the homeowners. This lead to late delivery, poorly reviewed performance and then the 7% price increase. Future homeowners started considering the prices charged by CMR to be too high. We could therefore expect lower level of sales and revenues in the future. Alternatives generation While we will mostly concentrate our alternatives on the three points mentioned above, we will also include other alternatives, even if they may not be realistic or feasible in the short/medium term. 1) Review customers change requests: We obviously cannot stop change requests and modifications made by the customers. The company and its employees pride themselves by offering flexibility and higher customers satisfaction. It would be therefore self-destructive for CMR to put a term to these showrooms. Any person also deserve to have reasonable alternative options when they are buying an object, a home more than anything. However, there is a clear misunderstanding between CMR and Blackstone Homes regarding the fixed allowances for kitchen ($6,000) and the final cost of the installation. In order to solve this issue, it may be a good thing to review the billing part: Instead of having the installation costs forwarded to the contractor, all expenses over $6,000 (parts + labor) should be charged to the homeowners themselves; customers can have access to all options made available by CMR, whilst eliminating cost forwarding and the said related disagreements. 2) Software adaptation It seems clear from the text that the whole issue also come in part from InfoCentral. In order to remediate this issue, we would suggest that all account managers from CMR and employees involved with this software should meet and discuss its performance. What should be changed, what must be kept, or what should be included in order to fit everyones needs. However, for any IT solution to bring tangible additional value, it must be easy to use and should not impose additional work. It is supposed to simplify the work, not making it more complicated. 3) The right people on the right place Again, according to the text, there was a clear misunderstanding between both parties; it was even referred as a shouting contest, which is far from being a normal and healthy business relationship. The Key Account Managers (from both sides) could not agree on contract terms and conditions. It is technique use to pin point the correct gaps between expected levels of service and the actual level of service provided. Gap analysis has played a crucial role in planning asset within public sector and private sectors as well. This analysis also gives a summary of the impacts of the set qualities to the management. This analysis helps the management to recruit new staff in the future. The management can cross reference it to check if the candidate has met the requirement, so that they can achieve their future plans. The analysis is generally used at the macro level. It identifies the key performance area of the organisation. It is necessary to close the gaps in desired and current levels of skills by every organisation in order to be successful in market. Identified Gaps in Chutney Mary: Up-selling: Up-selling guides restaurant to achieve the target set by the management. In Chutney Mary the management had always given the target revenue for every month. From this employees will be aware of their their target market for their product and what exactly they have to up-sell. There is quite a big gap in up-selling. The gap can be closed by giving training to the employees. This to have a good knowledge about the product and will also help in up-selling the brand image of the restaurant. Time management: It is an important tool in the hospitality sector. Time is one of the factors that cannot be wasted by the management. Skill audit the employees are lacking in time management. It sis a new restaurant managers must plan and manage the time. Inability to this will lead to a greater loss of revenue and reputation of the restaurant. For closing this gap special training must be given to the employees regarding time management. Communication skills: Being a hospitality sector it is very important to know that communication skills plays a vital role. In Chutney Mary many of the employees are Asians and Europeans and they are lacking in English so management must be well aware of it. As Chutney Mary is a new restaurant and which serve dietary food so there will plenty of reviews from the customers so staff should be able to explain the concept. For closing this gap management should arrange special English classes for those employees who are not fluent in English. Product knowledge: Product knowledge is one the gaps identified in Chutney Mary. Every staff must aware of the product and brand standards. It can be close by undergoing proper training to the staff which enhance the employees to explain the customers about their product with ease and confidence which will result in up selling. The restaurant must focus on shortage of skills and their impacts on Chutney Mary. From the given skills audit we can find out what skills are the employees lacking in: Lacking in efficiency and effective communication skills. Product knowledge. Lacking in time management. Up selling skills and its techniques of the employees are not up to the mark. Impact of the gaps to the business and achievement of future plans and objectives of the organisation: Due to lack of skills within the staff the restaurant will not be able to achieve future targets. This may have an effect on the productivity and can lead to negative comments and perceptions of the customers. Poor product knowledge may create lower revenue even when the staffs are doing through up selling. This can lead to the down fall of the gross revenue. Thus making lower profit on the sales volume. Less knowledge about the product makes the staff aware of the highlights of the products. It may lead tension and lower self confidence while they are dealing with customers. Lack of skills development results in less productivity of the organisation and poor and unacceptable performance. Lack of communication skills results in higher guest complaints. It increases the number of unsatisfied guest, and thus to bad word of mouth publicity. Because of this the management fails to achieve their future goals. Lack of up-selling technique will result in loss of potential revenue that the company can make. Organisation may lose its business in the market sector. They also have a fear of losing their market position. Explanation of staff development plan: As per the results from the above staff development plan I have explain it in details the summary of these plan: Up selling skills training: Up-selling plays an important role in the organisation since it helps to provide the value of the product than the price verified for it. Up-selling sales report is put on the board for every month to know the whether the restaurant has gain more revenue or not. The restaurant has a target of doing up-selling of  £20,000/- and above within a month time. This will help to increase the sales by 30%. Various up-selling techniques are used by the trainer who are invited from outside. From the above calculation there was  £206 per hour of average increase in the productivity rate. Menu knowledge: having accurate Menu knowledge has a good impact on the employees. By giving the training about the menu, restaurant is hoping to achieve the target of 45% on sale within 3-4 months and from the above calculation it was observed that there was 10% increase in sales. Time management: Time management is done in order to maintain the standards and consistency of the quality of work. Employees must be aware of the importance of time. The organisation has to keep the prospective of events like Olympics 2012. Communication skills: Communication skills are important in any service industry. Customer should know the concept and is easier when the staff has good communication skill. From the above chart 70% of employees who lacked in the communication skill left the job. Training wise restaurant is improvising on training the staff regarding with their English language. Increase in staff retention: In Chutney Mary Staff retention program is not effective. From the above plan it was found that 70% of staff has left the job. Creating a friendly atmosphere and providing regular staff training the restaurant is seeking to cut down the employment cost by 34% by the year end. Summary of this plan: What has to be done: From the above development plan the restaurant is planning to train all the employees within the department. For achieving this, restaurant must follow the objectives which had prioritised in the staff development plan. Special training must be given to the following issues menu knowledge, time management, and communication skills and staff retention. Employees should be trained on the brand standards so as to achieve the objectives. For that restaurant must arrange staff meeting once in a week. And have feedback from the employees and must discuss about what has to be done in order to achieve the goals. Who is going to do it: the objectives are for all level of staff within the organisation. It is the managements responsibility for arranging the trainings and evaluating the staff progress. Performance appraisal or staff performance check can be useful for this purpose. If the employees are successfully trained and are able to achieve this objective they should be rewarded by the manager of the restaurant. When it is going to be done: November 2009 the restaurant starts the evaluating employees recruitment, whether the employees are happy with their job through employees feedback form. December 2009 the restaurant will start with their up-selling training program and it will be finish by the end of 3 months. These programs will be held on Fridays and Saturday for every 1 hour. Brand standards training will be held in November 2009 on every weekend for 1 hour. Time management training will be held on January 2010 and communication skills training will be held in February November 2010. The time duration for all the training is of 3-4 months making them aware of time period. The employees who are lack in communication skills in English will be trained throughout the duration of the program. All this training will be done at management level and at the lower level as well. How it is going to be done: In Chutney Mary management puts training session on the notice board and gives the reminder of the session on the contact number of the employee. Websites resources like brand awareness, customer recommendation helps the employee to grow. Why it is going to be done: It is done to develop the personnel skills of the employees. The restaurant will have efficient staff to provide quality service after completion of the training session. This will result in increase in productivity of the restaurant and customer satisfaction and will improve. The restaurant will have a new face in the market sector, and within the competitors. This will be helpful to achieve the competitive edge against the competitors. How much will it cost: Approximately  £200-300/- quotation has been allocated to achieve these objectives. A total expenditure of the training session which consists of staff expenses, trainer expenses, training room charges etc. will be approximately around  £1000 to  £1500/-. How will the plan benefit the business? Training on up-selling helps to improve the skills of the staff while up-selling the product to the customers and gain more profit. Knowledge of the menu is key tool for the staff so that they can recommend the needs to the guest. This will help the staff to improve their skills and knowledge. Through time management staff will become more efficient and productive and they can become focussed in their work. By using communication skill the staff is expected to interact with the guest and top level management with effective communication. The staff retention will help to identify whether the employees are satisfied with their job. It also creates friendly atmosphere and as a result in smooth running of the operation. It also ensures that employee cost will be reduced. This plan will help the business in getting more customers. It will also help to achieve competitive edge over the competitors and helps to creating a reputation within the market sector. While presenting the bill folder the comment cards are also presented and through comment cards we receive the customers feedback. For that we have mention certain ratings from 1 to 5. If the ratings are more than 5 staffs are rewarded. It also helps in checking the performance of the restaurant. Sales tracking system: Management keeps track on the sale through this system. In the system they compare the present and past sales and even do the break even analysis. The Shop The sound made Elizabeth jump. Yet again it had come from the shop next door. She frowned at the old, battered doors of the ancient antique store. It was as though the owner was renovating. The noise next door has been going night and day for the past three days, and because she lived at the back of her boutique she couldn’t get a wink of sleep. A truck pulled up next to the boutique, two extremely hairy and extremely tattooed men came out the back and began unloading boxes marked â€Å"summer drs† onto the footpath. After about ten boxes were stacked not–so–neatly next to Elizabeth’s feet the men got back in the truck and started the engines. â€Å"Wait a minute!† shouted Elizabeth. â€Å"You need to put these in my store!† â€Å"That’s your problem lady, not ours.† One of the men jeered, his friend laughed and slapped him on the back, and with that they were gone. He entertains himself by collecting snails, looking for beaver damns, and skiing. His personality does not allow him to depend on anyone for help. As time progresses and the other boys start the winter session he decides to enroll in the war. A couple of months later Gene receives a telegram from Leper saying he is in his â€Å"Christmas location† This is the first sign that Leper demonstrates that he needs people. He realizes the happiness, fear and anger have to be shared with others. Leper then tells Gene â€Å"I escaped† and he says he did it to please himself. He is starting to realize things that are going on in the world. He no longer lives in his bubble. Gene Forrester is a quiet, envious, introverted, and lonely young man. His insecurities are especially seen when Finny is around. There is a constant rivalry through Gene’s eyes between them. He always chooses to compete or argue with Phineas. The first example of his competitive personality is seen when Phineas asks him to jump out of the tree. Phineas says, â€Å"you were very good, once I shamed you into it. .. I am good for you that way. You have a tendency to back away from things otherwise.† and Gene states â€Å"you didn’t shame anybody into anything. As time progresses the rivalry increases and the tension in Gene’s head is gets worse. When Gene can not take anymore of Finny and his abilities his jealousy takes control and he jounces the limb. When viewed superficially, Joseph Conrad's Heart of Darkness is a tragic tale of the white man's journey into the African jungle. When we peel away the layers, however, a different journey is revealed - we venture into the soul of man, complete with the warts as well as the wonderful. Conrad uses this theme of light and darkness to contrast the civilized European world with the savage African world in Heart of Darkness.   Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚     In Heart of Darkness, Conrad uses light and dark to symbolize good and evil, respectively. Celiac Disease

Celiac Disease Celiac disease is a disease found in the small bowel. This disease does not have a cure. It can only be treated with a strict diet. There are four types of celiac, and all but one can be treated. The disease is a genetically inherited associated with the HL4 locus found on the arm of the chromosome six (schaffner,small-bowel and bacterial overgrowth 2006 pg. 99). This disease can cause a lot of problems with a patient if not treated properly. The proper treatment for most individuals is to go on a gluten free diet. A gluten free diet avoids wheat, rye, barley, and sometimes oats.Some symptoms are excessive diarrhea, smelly stools, cramps, and weight loss. The most accurate way to diagnose celiac disease is to do an upper endoscopy on the patient. An endoscopy is a procedure with a tube called a scope and a small camera on the end. The scope goes down the patient’s throat to look at their upper gastric region. A biopsy of jejunal mucosa would be done in the small intestine to send to pathology and determine if the patient has celiac disease. When a patient is on a gluten free diet and their body has not responded to it within two years they call this non-responders.Only five percent of individuals are non-responders. There is also called a refractory sprue this is when someone does not respond to the gluten free diet or has responded and over some time has slipped back and stopped working so the patient has the same symptoms as they did before. There are two types of refractory sprue. Type one is a normal population of intraepithelial lymphocytes. Type two is an premalignant population of intraepithelial lymphocytes based upon clonality analysis of t-cell receptors and immunophenotyping. Type one can never lead to type two, a patient either has one or the other.Type one also has a higher survival rate of a five year study. This disease is a slowly growing disease. Now we find in some areas in the populations that it can be higher than one i n two hundred and fifty people. Celiac disease is a digestive condition triggered by consumption of the protein gluten. Many individuals will experience an immune reaction to the gluten that is digested. These proteins are mainly found in bread, pasta, and many different foods that contain wheat, barley and rye. Some foods that contain gluten that are over looked are brown rice syrup, energy bars, mitation seafood, processed luncheon meats and many more. When we experience a patient that doesn’t obey the gluten free diet they could do cause damage to the inner surface of the small intestine and not have the ability to absorb certain proteins that the body needs. There are four types of celiac disease. Type one has an increase in T- cell receptor intraepithelial lymphocytes. The symptoms of this type are malnutrition and weight loss with no symptoms of gastrointestinal symptoms. Type two has enlarged crypts along with the intraepithelial lymphocytes.Type one and two are the on ly types that can be treated. Type three is present in all symptomatic patients, but many patients with this lesion are asymptomatic. Type four is irreversible and is found in patients who do not respond to gluten withdrawal and in patients with lymphoma (schaffner,small-bowel and bacterial overgrowth 2006 pg. 100). The one way to diagnose a patient with celiac is to do an upper endoscopy and take a biopsy. To get the most accurate diagnosis is to biopsy the jejunal mucosa in the small intestine. They can also do serologic testing. It tests the IgA antigliadin antibody levels.They mostly use this test to see if the gluten free diet is working. Patients with celiac disease have a higher chance of lymphoproliferative disease and gastrointestinal then the average person. Studies have proven that a person with celiac is more likely to develop other autoimmune disorders like type one diabetes mellitus, and connective tissue disease. Mothers that have not been diagnosed with the disease c ould have low birth weight newborns and preterm birth compared to the mothers that have already been diagnosed with the disease. The treatment for celiac disease is gluten free diet.Gluten is a protein in a variety of foods. Oats have no gluten, but often cause problems for patients. They should limit the amount of oats they consume a day about fifty to sixty grams. The number one rule in this diet is to avoid wheat, rye, and barley completely. Always make sure the label is read on items carefully, they find ways to put gluten in different places. The safe things to eat are fresh vegetables, fruits, meats, fish, and eggs. Almost all foods in grocery stores have foods that have been mixed with gluten containing grains, additives, and preservatives.This makes it hard to find things in grocery stores. Now that this disease has become more noticeable in the communities, patients can find gluten free items in selected grocery stores. With our economy today some patients are unfortunate a nd cannot afford this gluten free food. For example a loaf of bread with gluten cost no more than three dollars, for a gluten free loaf of bread it cost six. This is doubling the prices on food. So I am sure there are organizations out there that will help someone provide food for themselves or for their children.Everyone has to watch out for cross contamination if our bread was cooked in the same baking pan as the gluten free bread was the patient with celiac disease would probably have a reaction to this. This is a very hard diet and it takes a lot of getting use too. If the patient looks at this diet in a positive way it will come easier to them. Approximately seventy percent of patients have noticeable clinical improvement within two weeks (Ciclitira, MD, PhD, FRCPJ; Lamont, MD; Grover, MD MPH; Up to date, 2012 www. uptodate. com).Once a patient reaches remission stage in the diet, some patients are able to consume little amounts of gluten and tolerate it and some have to stay o n the diet from then on. A patient should be evaluated four to six weeks after starting the gluten free diet to have a complete blood count, folate, B12, iron studies, liver chemistries, and serologic testing to see how they are doing and making sure nothing else is wrong. A gluten free diet is the only treatment for celiac disease. So when a patient is a non-responder to the gluten free diet they could be in some danger.This is a rare thing that happens only in five percent are non-responders. Patients who do not respond fall in five categories. * Patients with poor compliance or inadvertent gluten ingestion * Patients with clinical or histologic features that overlap with celiac disease but are caused by other disorders * Patients with concurrent disorders * Patients with refractory sprue * Patients with ulcerative jejunitis or intestinal lymphoma The most common reason for non-responders is poor compliance or inadvertent gluten ingestion. These patients need to go to a specificit y trained dietitian.Patients with concurrent disorders should be considered in patient who despite apparent compliance, continue to have symptoms or do not have histologic improvements. Refractory sprue has two types. Type one is when there is a normal population of intraepithelial lymphocytes. Type two is when there is an aberrant or premalignant population of intraepithelial lymphocytes base upon conality analysis of t-cell receptors. Patients with type one have less severe presentation and a more better prognosis than patients with type two disease.
